We want to hear from you! Every two years Long Valley Health Center conducts a survey to ask you (our community) your thoughts. What are your health concerns? What are your health goals? Where do you and your family go when you need medical help?

By answering the questions on this survey you help us understand your health needs so we can develop programs and services to meet those needs.
