At Long Valley Health Center, we focus on taking a patient-centered approach to health care. A medical home is a team approach to helping you understand and manage your health. Your medical home team includes your health care Providers and Care Team, others in your life who support you, and – most importantly – you.

Long Valley Health Center was initially recognized as a PCMH in 2018 and was the first clinic in California to receive the Distinction in Behavioral Health Integration recognition.

What is a PCMH?

The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams.
