Long Valley Health Center offers health care services to all individuals in Mendocino County and the surrounding areas.
Who can receive care at Long Valley Health Center?
Anyone! Long Valley Health Center offers services to individuals of all ages, regardless of ability to pay. Additionally, the following methods of payment are accepted:
Partnership Health Plan recipients who are assigned to our physicians as their primary healthcare provider
Medi-Cal recipients who request to see one of our Medi-Cal-approved physicians as their primary healthcare provider
Most privately insured individuals who choose Long Valley Health Center as their health care provider and Medical Home
Uninsured individuals will be charged for services based on an income sliding scale. There will only be a minimum charge per visit.

What is a Community Health Center?
Community health centers like Long Valley Health Center are patient-focused organizations that help people who have limited access to health care. As a community health center, Long Valley Health Center plays a key role in providing care to meet growing health needs and offering preventive services in North-Central Mendocino County. It serves as a trusted source of primary care for thousands of patients in Northern California.
Every two years, Long Valley Health Center conducts a Community Health Needs Assessment to identify local health care needs. This includes a survey where community members share their thoughts on what services are needed most. The feedback helps the Board of Directors create programs and services that better support the community.

• Individual health coaching and case management by our Case Managers and Behavioral Health Team

• Grocery Shopping and Nutrition Group
Video Medicine
• Primary care telehealth and telepsychiatry available
Ongoing Patient Surveys
• To provide opportunities for patient feedback so we can hear your compliments and concerns
Patient Portal
• Online access to your records via computer or phone
