Case managers meet with patients to understand the unique needs and circumstances of individuals and identify potential barriers to care.  Through individualized care plans, case managers can address barriers to healthy living. 

Through coordinated and personalized care, case management contributes to improved health outcomes by addressing both medical and non-medical factors affecting the individual.

Case managers outline specific goals, interventions, and timelines, ensuring a coordinated and targeted approach to healthcare.
