Long Valley Health Center Awards

HRSA awards Community Health Quality Recognition (CHQR) badges that recognize Health Centers that have made notable quality improvement achievements in the areas of access, quality, health equity, health information technology, and COVID-19 public health emergency response. Long Valley Health Center has received awards every year since the HRSA CHQR program started in 2015.

2024 HRSA CHQR Awards

LVHC was awarded 3 Community Health Quality Recognition (CHQR) awards this year - Addressing Social Risk Factors, Advancing HIT for Quality and PCMH Recognition. LVHC received these awards because:

  • We increased the percentage of patients who received case management services in the last two years from 3% to 10%

  • We use Health Information Technology such as ePrescribing, Telehealth and a Patient Portal to increase access to quality care

  • LVHC is a Patient Centered Medical Home with a Distinction in Behavioral Health
