Delivering a Big Impact in a Small Community
Located in Laytonville—a community spanning just five square miles with a population of 1,152—we punch far above our weight. Last year, we proudly provided care to 2,736 people. Our comprehensive range of services includes medical, dental, behavioral health, chiropractic care, case management, transportation, and more. We’re also expanding to meet our community’s needs with the addition of optometry services.

2024 HRSA CHQR Awards
HRSA awards Community Health Quality Recognition (CHQR) badges that recognize Health Centers that have made notable quality improvement achievements in the areas of access, quality, health equity, health information technology, and COVID-19 public health emergency response. Long Valley Health Center has received awards every year since the HRSA CHQR program started in 2015.
In 2024, LVHC was awarded 3 Community Health Quality Recognition (CHQR) awards - Addressing Social Risk Factors, Advancing HIT for Quality and PCMH Recognition.
LVHC received these awards because:
We increased the percentage of patients who received case management services in the last two years from 3% to 10%
We use Health Information Technology such as ePrescribing, Telehealth and a Patient Portal to increase access to quality care
LVHC is a Patient Centered Medical Home with a Distinction in Behavioral Health
