LVHC receives HRSA Addressing Social Risk Factors Award for 3rd year in a row

For the 3rd year in a row, Long Valley Health Center has received the HRSA Addressing Social Risk Factors Award. The annual Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees that have made quality improvement achievements in access, quality, equity and health IT. In 2024, we were also awarded the Patient Centered Medical Home (PCMH) Recognition and Advancing HIT for Quality badges.

LVHC earned this badge because we have increased the percentage of patients who received case management services in the last two years from 3% to 10% of our patients. Thanks to our growing helpful Case Management Team we are able to support patients and their families with these services and more:

  • Assist with Medical, Dental, and Behavioral Health referrals and transportation

  • Connect you to local programs and services

  • Grocery and support groups

  • Job applications and search assistance

  • Vocational training

  • Enroll and assist with insurance applications in both Medical-and Covered California

  • Assistance with Medicare questions

  • Enroll in County and State Benefits such as WIC and CalFresh

  • Assist with State and Federal Disability Services

  • Bill paying financial application assistance

  • Housing applications and search assistance

  • Assist with In Home Support Services applications for both care giver and recipient

  • Assist with transitional care (home to care facilities and back home)

  • Assistance with dealing with drug and alcohol misuse
